| Smile Plan for small business is a reduced fee-for-service dental plan offered to local businesses in Woodbury and its neighboring cities. It is designed to make dentistry more affordable for you, your employees and associated families. Dental services are provided by one of our dentists at Eagle Valley Dental. AFFORDABLE MONTHLY PREMIUMS - Smile Plan gives you, your employees and associated families hundreds of dollars worth of services and discounts for an extremely affordable price. Take a look at the affordable monthly premiums.
Each Dependents: $5.00 / month Effective Date - Coverage becomes effective on the 1st day of the month after the day we receives your application and first month premium payment. Renewal Payments - Once you are accepted into the Plan, you will be billed on a monthly basis. To cancel any member from the plan, you will have to send a written request to the plan administrator that clearly identifies the name and social security of the member(s). Upon receipt of this request, the membership of the corresponding member(s) will be cancelled effective the following month from the date the request was received. IMPORTANT PROGRAM AND PROVIDER INFORMATION - Smile Plan is NOT insurance. It is a discount dental program provided by Eagle Valley Dental. The discount is applied to only procedures offered by Eagle Valley Dental. Fees may change from time-to-time. A Member’s participation in this program is governed by the terms of the Membership Agreement. HOW TO APPLY - Refer to the above plan description and membership agreement for benefits, premium costs, terms and conditions as you fill out the enrollment form. Make a check payable for the total amount of the first month premium due payable to: Eagle Valley Dental. Mail the completed Application form with your check to: Eagle Valley Dental, 2110 Eagle Creek Lane, Woodbury, MN 55129. An acceptance letter will be sent to your business shortly. Once you receive this letter, you, your employees and associated family members are entitled to all the discounted dental services in the program. HOW TO RECEIVE THE DISCOUNT - To receive these discounts, Contact Eagle Valley Dental at 651-998-1008, identify yourself as a member of Business Smile Plan program and setup an appointment. Emergency visits are generally scheduled for the same day or next business day, and regular appointments can usually be scheduled within a short period of time. After completion of your dental session, you will pay according to the special affordable rates of the Smile Plan. Questions - We're Only a Phone Call Away.
