Insurance company allows a certain limit of dental expenses per year. Based on the type of treatment the benefits vary. The insurance company
pays a percentage of the expense depending on the type of service provided. The range of benefits depends solely on what your employer
wishes to purchase. Some plans may cover as little as 30% or as much as 100% of dental services, with most falling in the 40% to 80 % range.

The financial obligation for dental treatment is between you and our office. The insurance company is responsible to you, and not to our office.
We will be happy to assist you in any way that we can. Once your carrier has paid the claim, any difference will be due upon receipt of our
statement. If for any reason, we have not received your insurance carrier's payment 60 days after the claim, the remaining balance will be due
and payable by you.

The following is a list of major dental insurance, dental fee-for-service and supplimental dental plans we accept.  It is likely that we accept your
insurance plan, even if it does not appear on the list below.   It is also possible that you carry one of the plans from the following insurance

We accept most dental insurance plans, some of which are listed below.  Also, we'll file
your insurance forms as a courtesy to you. It is important, however, that you read your
policy so you understand the benefits you're entitled to, as the success of your dental
insurance depends on your full understanding and cooperation in filing claims.

For patients who do not have dental insurance we offer a dental discount program
called SmilePlan.  Please visit our

It is our pleasure to assist you by completing your dental claim forms. As a courtesy , in
addition to filing the claim, we may file a pre-authorization to get estimated co-payment
from your insurance company. Please understand that this is only an estimate, and it is
based upon the information available to us by your insurance company.

Most patients are unaware of their dental benefits. They feel that similar to medical
insurance they will only have to pay a fixed office co pay during their dental visit. Dental
benefits vary depending on the procedure being done. Dental procedures are generally
categorized into preventative, basic and major. Coverage with each category varies
depending on your insurance plan.
