Personal Smile Plan is an in-house reduced fee-for-service dental plan we offer to our patients who do not have dental insurance.  It is designed to make
dentistry more affordable for you and your family.  Dental services are provided by one of our dentists at Eagle Valley Dental.  

MEMBERSHIP BENEFITS - as a member of Personal Smile Plan you receive the following discounts

  • One FREE initial exam with your initial cleaning           (Exams do not include X-Rays or cleaning procedures which are charged at 20% discount)
  • Two FREE exams with your annual cleanings              (Exams do not include X-Rays or cleaning procedures which are charged at 20% discount)
  • FREE emergency exams all year round                         (Exams do not include X-Rays or cleaning procedures which are charged at 20% discount)
  • 20% OFF all non-cosmetic procedures offered by Eagle Valley Dental

AFFORDABLE ANNUAL PREMIUMS - Personal Smile Plan gives you and your family hundreds of dollars worth
of services and discounts for an extremely affordable price. Take a look at the affordable annual premiums.

    Individual:                  $   70 / year
    Family:                        $140 / year
    Senior Citizens:        $   35 / year

EFFECTIVE DATE - Coverage becomes effective on the day you sign the application and pay the first year premium.

RENEWAL & PAYMENTS - Once you are accepted into the Plan, you will be billed on an annual basis. You can request to cancel your membership by
contacting one of our staff.  Upon receipt of your request, the membership will be cancelled effective your annual renewal date.  There will be no refund of
any and all annual premiums paid.  

PROGRAM AND PROVIDER INFORMATION - Smile Plan is NOT insurance. It is a discount dental program provided by Eagle Valley Dental only.  The
discount is applied to only procedures offered by Eagle Valley Dental.  The plan is not available for any other dental facility.  Fees may change from time-to-
time.  A Member’s participation in this program is governed by the terms of the Membership Agreement.

How to apply - To apply, please complete the personal Smile Plan Application and bring it with you to your first appointment along with the membership
fees.
